These problems may last over a long period of time, and may include heart disease, diabetes, reduced mobility, difficulties with the activities of daily living, memory loss and other medical, emotional and social issues.
To meet the extensive needs of this population, Crozer-Keystone Health System developed the Geriatric Evaluation and Management (GEM) program. Led by a nationally recognized geriatrician, William S. Zirker, M.D., our team of specially trained health care and social service professionals work with primary care physicians throughout our community to provide consultations and comprehensive assessments for seniors. Our unique team approach to geriatric medicine enables us to provide quality health care to older adults and their caregivers.
Services Provided
A Comprehensive Evaluation
A comprehensive evaluation customizes the patient's plan of care. Each patient receives an extensive geriatric assessment by our team of specialists, which includes a board-certified geriatric physician, a certified geriatric nurse practitioner and a social worker. The assessment includes a physical exam, an evaluation for memory loss, depression, day-to-day functioning, walking, nutrition and medications. Social support and community services are also assessed.
A Treatment Plan
Following the assessment, our medical team will discuss its findings and recommendations with the patient, their family, or other caregivers. The completed GEM treatment plan will include medical care, emotional counseling and social work services.
The completed plan will be presented to the referring primary care physician for continuity and to engage the physician in the patient's plan of care. If the patient does not have a primary care physician, our team will carry out the plan to ensure that the patient's needs are met.
As part of our commitment to providing ongoing medical care, the patient will be followed and connected to the appropriate resources. This may include accessing community services such as adult day care, housing, in-home services, meal programs, respite care, transportation, and social activities.
When Daily Activities Become Challenges
As we age, everyday activities may become challenges. The GEM program helps seniors who are experiencing any of the following problems:

- getting to and using the bathroom
- preparing meals
- eating without help
- getting out of bed or a chair without help
- Difficulty or trouble managing bills or household tasks, such as housework and laundry
- Difficulty shopping
- Difficulty with medicines: can't remember when, or which ones, to take
- Falling, or fear of falling
- Memory problems
- Worrying, sadness or loneliness
- Accidental urination
