A Message from Brad Prechtl, President, Crozer-Keystone Health Network …

 

As you know, our community’s primary care physicians make a significant contribution to patient wellness and to the ability of Crozer-Keystone Health System to meet its mission and objectives. For most of our patients, the primary care physician the first contact with our entire network of physicians and hospitals. With this in mind, I am pleased to announce that I am working with Crozer-Keystone’s Marketing and Public Relations Department on the launch of a campaign to raise awareness of our hospitals’ family practice physicians and internists.

         

The following campaign materials are in production:  

 

   Three new printed directories of primary care physicians -- for DCMH, Taylor and Crozer/Springfield/Community -- will include names, addresses and phone numbers of all primary care physicians with privileges at these hospitals. The directories will be distributed throughout our hospitals, by all employees who do community outreach, through our Call Center, and as a response mechanism for many of the health system’s marketing mailings. We will also post the directories online, linked with each hospital’s home page.

   Print ads in local newspapers will direct potential patients to visit our website or contact the Call Center to request a directory or find a primary care physician.

   Postcards mailed directly to households in the appropriate zip codes will carry the same messages as the print ads.

         

This issue’s “Quality Corner” reports on the FY 2008 results of three CKHN Quality Improvement initiatives: Influenza Immunization, Physician Quality Reporting Initiative (PQRI), and Patient Safety. A number of important goals have been achieved thanks to the efforts of CKHN physicians and staff. Meanwhile, the CKHN Quality Improvement and Medical Affairs Committees are finalizing our goals for FY 2009; I’ll keep you posted.

Dr. Breslow, a graduate of the Philadelphia College of Osteopathic Medicine, completed his residency in the Crozer-Keystone family medicine residency program. He served as chief resident for family medicine at Crozer-Chester Medical Center.  Board eligible, he is a member of the American Academy of Family Physicians. His professional interests include sports medicine, diabetes, hypertension, hyperlipidemia and health care for all ages. Dr. Breslow also serves as a physician at Elwyn Institute. Dr. Delfin, a graduate of the Philadelphia College of Osteopathic Medicine, completed her residency in the Crozer-Keystone family medicine residency program, where she was honored with the Behavioral Science Award. Board eligible, Dr. Delfin is a member of the American Academy of Family Practice. Professional interests include the care of women and children, diabetes and preventive medicine. Dr. Feinberg, a graduate of the Philadelphia College of Osteopathic Medicine, completed his residency in the Crozer-Keystone family medicine residency program, where he was named Family Medicine Resident of the Year. He is a house physician at Springfield Hospital. Board eligible, Dr. Feinberg is a member of the American Academy of Family Practice. Outpatient Rehab at Crozer-Keystone Human Motion Institute (HMI) ...
When a patient needs a referral for outpatient rehabilitation, remember that Crozer-Keystone HMI provides comprehensive care including:

   Assessment and treatment of injuries of the hip, knee, ankle, foot, shoulder, elbow, wrist, and hand

   Evaluation and treatment of other musculoskeletal problems, such as sports-related injuries, occupational injuries, and post-surgical care

   Education and injury prevention for home, work or school

   Evaluation, treatment and prevention of neck and back injuries

   Specialized services:  vestibular rehab, pelvic floor rehab, aquatics, falls assessments, and lymphedema therapy

Delaware County Memorial Hospital (DCMH) earns Primary Stroke Center status ...
In May, DCMH became the first hospital in Delaware County to earn certification as a Primary Stroke Center by the Joint Commission on the Accreditation of Healthcare Organizations (JCAHO).  Stroke certification guidelines require applicants to have a stroke team and neurosurgical services available around the clock, written diagnosis and care protocols, close coordination between the emergency department and emergency transport, and a dedicated stroke unit, in addition to specific clinical expectations.

The Center for Diabetes at Springfield Hospital is an American Diabetes Association-recognized outpatient diabetes program ...
The center provides outpatient education for individuals with new onset and uncontrolled diabetes mellitus, or for those who desire intensive control. Special instruction is available for patients with prediabetes and gestational diabetes and those who need insulin pump therapy. Education is offered at Springfield Hospital and the Crozer Medical Plaza at Brinton Lake. The Center is currently accepting new patients and will work to accommodate each patient’s specific needs. CKHN Quality Corner: 2008 Quality Improvement year ends with many lessons learned ...

Fiscal Year 2008 is behind us. Many goals in the CKHN Quality Improvement arena were achieved, the goals not achieved will be evaluated for inclusion in 2009 or new directions may be sought for how to achieve them.  While 2009 will bring a new set of challenges, we thought it was important to report the results for FY 2008.
