Crozer-Keystone Health System's Home Health programs provide care in the comfort of patients’ homes. These patients may be newly discharged from the hospital, referred directly by physicians’ offices or in need of skilled care.
Health care and supportive skilled services are provided through a team approach that incorporates the quality care that has been a Crozer tradition. The Home Health team, a group of highly skilled professionals, works in cooperation with patients’ primary care physicians to formulate an individualized plan for care.
Will I still see my doctor?
Our registered nurses and therapists coordinate your home care services under your physician’s guidance. Even though we keep your physician up-to-date on your home care progress, you will continue to make regular office visits.

“There’s no place like home” is a popular saying. Often, patients recover more quickly in the familiar and comfortable surroundings of their homes.
Through the use of a hospital-based home health care program, patients and their families are assured that a highly skilled professional staff provides care for individuals or those making the transition from hospital to home.
Home health care may shorten or eliminate the need for an inpatient hospital stay or may help the patient delay or avoid nursing home placement.

Mother-Baby Connection
Our special Mother-Baby Connection provides support for recently discharged mothers and infants, lactation-certified nurses for mothers who choose breastfeeding and phototherapy at home so infants can go home when their mothers do.
A patient’s family member, friend, physician or other health care professional may request home care services from Crozer Home Health. The Crozer Home Health staff will gladly work with anyone who is interested in home care services to evaluate patient needs and eligibility and formulate an individualized plan of care. Payment for Home Health Services
Our home care intake specialists will make all of the necessary arrangements and do a full evaluation of your insurance coverage prior to your first home care visit.

Home health patients have access to a nurse by phone 24 hours a day, seven days a week.
Computerization – our Home Health professionals carry laptop computers to allow them to keep your health records up-to-date and communicate electronically with other members of the Home Health team.
